Living Well with a Chronic Condition will provide tools that can be used on a daily basis to manage your health conditions, such as diabetes, breathing problems, depression, anxiety, heart conditions, and arthritis.
Self-management techniques include:
- Action planning
- Using your mind to manage symptoms and emotions (distraction, positive thinking, relaxation)
- Fall prevention, improving balance
- Ways to maintain and improve strength, flexibility, and endurance
- Making healthy food choices
- Maintaining a healthy weight
- Effective communication with doctors, family, and friends about your condition

Chronic Condition Workshop Registration
For your convenience, we are offering multiple ways to participate. Registration is required. There is no fee, just a commitment to participate throughout the six weeks.
